For nearly 30 years, Alzheimer's Family Services Center (AFSC) has helped improve quality of life for families challenged by Alzheimer's disease (AD) or another dementia through services tailored to meet individual needs. Licensed by the California Department of Public Health, AFSC is the only Alzheimer's Day Care Resource Center in Orange County. AFSC is also proud to be affiliated with the Neurosciences Center of Excellence at Hoag Memorial Hospital Presbyterian in Newport Beach. A comprehensive center for the care and support of families affected by AD or another dementia, AFSC meets the needs of persons from the earliest to most advanced stages of Alzheimer’s disease or another dementia through a continuum of dementia-specific adult day health care, caregiver support, and education services.

DEMENTIA-SPECFIC ADULT DAY HEALTH CARE SERVICES are provided in a state-of-the-art, dementia-specific facility with a secure perimeter and a high 1:5 direct care staff-to-participant ratio. Service include Multi-Disciplinary Assessment and Care Planning, Health Monitoring and Medication Management, Cognitively Stimulating Games, Individualized Computer-Based Memory Stimulation, Creative Arts, Physical, Occupational, and Speech Therapy Maintenance Exercises, Balance and Mobility Training Activities, Daily Meals and Transportation to/from the Center that is arranged and/or provided.

CAREGIVER SUPPORT SERVICES include Information and Referral, Care Management and Consultation, Short-Term Solution-Focused Counseling, Support Groups, and JumpStart, a unique education and support group for early stage individuals and their care partners.

EDUCATION: Workshops and classes are available on a variety of dementia-related topics, such as diagnosis, treatment, and care, behavior management, communication and cognitive changes, and legal and financial planning.

Caregivers Report Benefiting from AFSC Services

AFSC adult day health care, caregiver support and education services are helping caregivers better manage the challenges of caring for a loved one with dementia, as indicated in recent evaluation surveys.

  • 89% of caregivers responding to the FY 08-09 adult day health care survey reported finding it easier to continue caring for a loved one since enrollment. Additionally, 88% reported being less stressed.
  • 100% of caregivers responding to the FY 08-09 support group survey reported being less depressed and better able to manage a loved one’s care as a result of attending.
